Purpose: Safety culture in teaching laboratories is critical in ensuring instructor and student safety. This research will explore current safety culture in public schools, with the goal of identifying best practices and action items to strengthen the safety culture. 

Eligibility: To be in this study, you must be a science faculty member (middle or high school) who uses chemicals for laboratory experiences or science demonstrations as part of their instruction. 

Participation: During the survey, you will be asked to respond to questions about your practices, opinions, and understanding. The completion of this survey will take approximately 15 minutes. 

Risks of Participation: The risks of participating in this study are minimal, no more than everyday life. Because of the implications of safety, there is a small risk of anxiety when completing the survey. 

Benefits of Participation: I do not expect the study to benefit you personally. Your assistance in this project will help form an understanding of the current state of secondary school safety culture, which may have indirect benefits to you. 

Compensation: There is no compensation for participation in this survey.

Voluntary Participation: Your participation in this study is completely voluntary. You may stop or withdraw from the survey at any time or refuse to answer any particular question without it being held against you. Your responses are anonymous. Your decision whether or not to participate will have no effect on your current or future relationship with anyone at your institution. Refusal to participate in this study will not be penalized and you will not lose benefits to which you would be otherwise entitled. If you opt out of the survey after having started the survey, responses previously submitted will be discarded. Because surveys are anonymous, once a survey has been submitted, individual responses cannot be discarded. 

Respondent Privacy: All survey responses are anonymous. No personally identifying information will be made public in any products generated from the study.
